パソコン、スマートフォン、タブレット
インターネットへの接続が必要です。
個人情報を入力するため、可能な限り、ご自身のパソコンで登録を行なってください。
パソコンからの出願登録をお薦めいたします。
PDFの表示・印刷には、アドビシステムズ社が配布しているAdobe Readerが必要です。インストールされていない場合には、こちらのサイトよりダウンロード（無料）してください。
博士後期課程、薬学博士課程を志望の方は、研究業績・志望理由・研究計画がWordファイルのため、Wordファイルが編集できるソフトが必要です。
書類提出の際にスマートフォンで撮影した画像をアップロード、またはスキャナを利用することがあります。
スマートフォンを利用する際には、画像補正や傾き調整、複数画像をPDFに保存できる機能のある、以下のアプリを推奨します。

PC, smartphone, tablet
An internet connection is needed for the procedure.
Please register on your own computer if possible to enter personal information.
We recommend that you register your application from your computer.
Adobe Reader distributed by Adobe Systems Incorporated is required to display and print PDFs. If it is not installed, please download it (free of charge) from this site.
Those who wish to apply for the doctoral program in Pharmaceutical Sciences or the PhD in the Pharmaceutical Sciences need software that can reliably edit Word files because the research achievements, reasons for applying, and research plan must be submitted as Word files.
When submitting documents, you may upload images taken with a smartphone or a scanner.
When using a smartphone, we recommend the following apps that have image correction, tilt adjustment, and the ability to save multiple images to PDF.

Webカメラ・マイク
口述試験ではzoomで行われます。そのためWebカメラが設備されているか、Webカメラを接続する必要があります。
また、マイク機能も必須となりますので、確認の上、ご用意ください。
Webcam / microphone
The oral exam is conducted via zoom. Therefore, it is necessary to have a webcam or connect a webcam.
In addition, the microphone function is also required, so please check it before in advance.
メールアドレス
出願にはメールアドレスが必要となりますので、事前にメールアドレスをご用意ください。
ドメイン指定受信を設定されている方は、「@yakugaku.online-exam.jp」「@mol.f.u-tokyo.ac.jp」からのメールを受信できるように設定を追加してください。
E-mail address
You will need your e-mail address to apply, so please have your e-mail address ready in advance.
If you have set the domain for a specified reception, please set it up so that you can receive the mail from "@ yakugaku.online-exam.jp" and "@ mol.f.u-tokyo.ac.jp".
必要書類
必要書類は、発行まで時間を要する場合があります。早めに準備を始め、出願前には必ず手元にあるようにしておいてください。
志望課程によって必要な書類が違うため、必ず募集要項をご確認ください。

Required documents

受験用顔写真データ
３ヶ月以内に撮影した正面上半身脱帽・無背景で顔がはっきりと認識出来る画像データ（JPEG,PNG,GIF,BM）。
スマホ等での自撮画像でも可。サイズは20MBまで
Face photo for the entrance examination
Image data (JPEG, PNG, GIF, BM) taken within 3 months with the front upper body uncapped and the face clearly recognizable without background.
Selfie images on smartphones etc. are also acceptable up to 20MB in size.
検定料
出願申請の際に、検定料支払証明書が必要になります。
申請する際は、振込を完了させてください。
振込方法は【銀行振込】又は【コンビニエンスストアでの払込】、【ペイジー対応ＡＴＭ、ペイジー対応ネットバンク、ネット専業銀行での払込】若しくは【クレジットカードでの払込】のいずれかです。 詳細はこちらをご覧ください。
検定料に関しては、募集要項をご確認ください。
※ 検定料支払証明書（銀行やクレジットカード等で支払ったことがわかるもの。
電子媒体で存在しない証明書については、 スマホ等のカメラ で撮影した画像データ（JPEG,PNG,GIF,BM））。サイズは20MBまで。
Examination fee
A certificate of examination fee payment is required when submitting the application.
Please complete the transfer before applying.
The transfer method is either [Bank transfer] or [Payment at a convenience store], [Pay-easy ATM, Pay-easy online bank, online bank] or [Credit card payment]. Please see here for the detail.
Please check the application guidelines for the examination fee.
※ Examination fee payment certificate (a paper slip that shows that you paid with a bank or credit card etc.)
For certificates that are not issued as electronic media, image data (JPEG, PNG, GIF, BM) taken with a smartphone camera and the size of up to 20MB are acceptable.
